Registration with the practice
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill.
All new patients are offered a health check with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further information.
You will also need to complete a registration form (PRF1) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
To register please visit the surgery with the following completed form:
- PRF1 Registration Form (also available from reception)
Alternatively, we are using a new online service called Register with a GP surgery that makes it easy to register with our surgery.
You can also scan this QR Code
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
We are currently working on a backlog of registrations and is currently taking 2-3 weeks. You will be notified via the NHS App, email or text message once you are registered. If you do not use the NHS App, email or text messages you will be sent a confirmation by post.

Find Your NHS Number
Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.
This online tool will help you find your number if you do not know it.
Please Note
You do not need to know your NHS number to use NHS services, but it can be useful to have it.
Other ways to get your NHS number
If you cannot get your NHS number online you can:
- find it on any letter from the NHS like a prescription or appointment letter
- call your GP surgery and ask them for your number
GP Patient Choice Scheme
The practice has signed up to provide urgent appointments and urgent home visits to patients who are resident in Calderdale but are registered with a practice taking part in the National Patient Choice Scheme. These means that patients who are registered at one of the pilot sites can access urgent appointments or, if clinically indicated, have a home visit if they are too unwell to travel to their registered practice during weekday daytime in hours (8.00am to 6.30pm).
The Patient Choice Scheme is to be piloted in the following areas:
Central London (City and Hackney Teaching PCT, Tower Hamlets PCT and Westminster PCT)
Nottingham City PCT
Manchester Teaching PCT
Salford PCT
(PCT = Primary Care Trust)
If you are registered with a practice in one of the above areas, and feel you need to access this service, please ring the surgery. You will be asked to give some details to reception and you will receive a telephone call back from the on-call doctor who will triage your symptoms and decide if you need an emergency appointment, home visit or if there is a more appropriate way of receiving treatment (e.g. Accident and Emergency).
Please try and help us by ringing as early as possible should you wish to access this service. This enables us to plan the day more effectively and to try and keep the time that other patients are waiting at the surgery to a minimum.
Once you have been seen, either at the surgery or at home, details of your consultation/medication etc will be transferred to your registered practice within 2 working days of the consultation.
Non-English Speakers
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
