New System for Reviewing long-term Conditions Online

Dear Patients,

We would like to make you aware of some changes happening at Church Lane Surgery.

We are moving to a system for reviewing long-term conditions online. When you require a review, you will receive an invite containing a link by e-mail or SMS text message, or receive an automated voice call asking you to contact the practice.

We aim to invite patients for their review in the month of their birthday.  Please note, that as we transition to the new system, for some patients this will mean they will be called less than or more than 12 months since their last review.

Please use our convenient online form on our website to update your contact details and make sure we are able to reach you Patient Record Update – Church Lane Surgery

Please click or tap on your link and complete your online review on a smartphone, tablet or computer. You can copy or type the link into a computer browser to use a larger screen.

These personalised reviews allow a single annual review for all common conditions.

Any conditions not included in the online system will continue to be reviewed in the practice as normal.

Your responses are saved automatically. If you can’t complete it in one sitting, just click on the link again later to continue. Feel free to ask a family member or friend to help you answer the questions.

After completing your review online, you may receive instructions if an appointment (for example, a blood test) is needed. Please follow the instructions to book appointments so that we have all the information needed to assess your conditions. Once all information is available, the practice team will look at your answers and follow up with you as needed.

If you do not want to complete the online review or prefer a face to face review in the practice please contact the surgery in the usual way to make an appointment.
