**Prescription Changes**

Please note, from the 1st Februrary 2018, the chemist will no longer be able to take prescription orders due to medication wastage. All repeat prescriptions must now be ordered through the GP Surgery. 

Orders can be made over The Church Lane Surgery website, dropped in on a right hand side prescription request form or in person at the surgery. Please note, we are unable to take prescription requests via the telephone. Please allow 2 full working days for a prescription to be ready. Your prescription will still go to your nominated pharmacy once ready.

Our professional team will ensure you receive the best attention at all times whether you’re attending a specialist clinic or a routine appointment. If you’re new to the area registration is easy and comes with the comfort of discretion and confidentiality assured.

From the 1st of April 2015 all practices are required to allocate a named GP to all of their patients. All of our patients have been allocated a GP. If you are not aware of your named GP, please contact the practice and we can inform you of who it is.

This does NOT prevent you from seeing any other GP.

 Hospital Transport

Please note Church Lane Surgery can only book hospital transport for your first appointment.

If you require a follow up appointment, the transport should be arranged through the hospital by yourself at the time of your hospital appointment.

You are eligible if you meet the following criteria:

If you have a medical condition which affects your mobility, or if you need the facilities of an ambulance crew in order to travel safe.

Please mention to the Doctor at the time of referral if you feel you may need transport.  

Patient Access

We have the facility to enable patients to book routine GP appointments online. Once some form of ID has been seen by reception, i.e, drivers licence, passport, utility bill etc, a unique login will be printed which will allow you to register onto the online booking service.
